Exosome Treatment for Hair Loss: Does It Work?

Exosomes are extracellular vesicles involved in cellular communication and tissue repair. Due to their regenerative, anti-inflammatory, and immunomodulatory properties, exosome-based therapies have recently gained attention as a supportive approach for complex scalp conditions and hair restoration.

Although still an emerging field, early clinical and preclinical studies suggest that exosomes may improve hair density, hair thickness, and overall scalp health, particularly when combined with regenerative procedures such as microneedling or radiofrequency microneedling (RF microneedling).

Introduction to Exosomes

Exosomes and Extracellular Vesicles

Exosomes are nano-sized extracellular vesicles released by cells that carry proteins, lipids, messenger RNA, and microRNA. These vesicles act as biological messengers that regulate cell signalling, immune responses, and tissue regeneration.

Through these signalling pathways, exosomes may influence:

  • Cellular proliferation
  • Tissue repair and regeneration
  • Inflammatory modulation
  • Stem cell activation

Because of these properties, exosomes are increasingly being explored for applications in regenerative dermatology, wound healing, and hair restoration.

Plant-Derived Extracellular Vesicles

Among the different types of extracellular vesicles under investigation, plant-derived extracellular vesicles (PDEVs) have gained attention due to several favourable characteristics.

These vesicles demonstrate:

  • Low immunogenicity
  • Antioxidant and anti-inflammatory properties
  • Cross-kingdom communication with mammalian cells
  • High stability and scalability for production

These advantages make plant-derived vesicles attractive candidates for therapeutic and cosmetic applications.

After decades of research into plant bioactive compounds, plant-derived exosome-like nanovesicles (PELNs) have emerged as promising natural bioactive ingredients. PELNs possess several biological and physical characteristics that support their therapeutic potential, including the ability to penetrate biological barriers and maintain stability within physiological environments.

Preclinical research suggests that PELNs can act directly as therapeutic agents in disease models. Additionally, they may serve as drug delivery carriers, allowing targeted delivery of anti-inflammatory or anti-tumour agents to specific tissues.

Rose Stem Cell-Derived Exosomes in Dermatology

Among plant-derived vesicles, rose stem cell-derived exosomes (RSCEs) have recently gained attention in dermatology and aesthetic medicine.

These vesicles demonstrate compatibility with minimally invasive delivery systems, including microneedling and radiofrequency microneedling, making them practical for real-world clinical application.

In addition to regenerative effects on hair follicles, rose stem cell-derived exosome therapy has also been investigated for its potential role in hair repigmentation.

A cross-sectional observational study evaluated the use of rose stem cell-derived exosomes in patients with gray hair. The study included 10 participants with visible hair graying, who received exosome therapy through various delivery methods. Patients underwent an average of 4.6 ± 1.3 treatment sessions, with repigmentation observed after approximately 2.4 treatment sessions.

The study reported that 60% of patients achieved at least a 50% improvement in hair repigmentation based on standardized outcome measures. Improvements in hair density and hair thickness were also observed in some participants. Importantly, no adverse events were reported during the study period.

Predictive factors associated with better treatment outcomes included:

Presence of androgenetic alopecia

Shorter duration of hair graying

Moderate baseline severity of depigmentation

Combining Exosome Therapy with Radiofrequency Microneedling

One of the most promising clinical applications of exosome therapy involves its use in combination with radiofrequency microneedling (RF microneedling).

Radiofrequency microneedling induces controlled micro-injury in the skin and scalp, which stimulates wound healing pathways and activates follicular stem cells. These microchannels also enhance the penetration and delivery of topical regenerative compounds.
Early clinical studies have reported that radiofrequency microneedling followed by topical exosome application may produce synergistic effects in hair regeneration.

Reported clinical benefits include:

Increased hair density
Increased hair shaft diameter
Reduction in hair shedding
Improved hair quality and growth

Prospective studies have demonstrated high patient satisfaction rates and favourable safety profiles, with no reported adverse events during follow-up periods of up to 10 months.

Similarly, a prospective study investigating the application of a stem cell-derived exosome regenerative complex following microneedling reported significant increases in both terminal and vellus hair counts, as well as improvements in hair growth and hair quality within a 120-day period.

It should be noted that some of these studies utilized animal-derived exosome preparations, which may differ from plant-derived vesicle systems currently being explored in dermatological applications.

However, due to the higher cost of exosome therapy, some patients may choose microneedling alone with regenerative scalp solutions such as ACSE HRLV.

Typical treatment protocols may involve:

  • Treatments twice monthly for approximately four months
  • Followed by monthly maintenance sessions

Treatment recommendations are individualized based on the patient’s hair loss pattern, scalp condition, and overall treatment goals.

Frequently Asked Questions

Exosome therapy is not recommended for patients who are pregnant, planning pregnancy, or breastfeeding, as safety data in these groups is limited.

Results are not permanent. Maintenance treatments are typically required to sustain improvements in hair growth and scalp health.

Side effects are usually mild and may include scalp discomfort, redness, or temporary irritation. As with any treatment, results may vary and some patients may experience limited improvement.

Complete reversal of grey hair should not be expected. Current studies suggest around 50–60% improvement in repigmentation in some patients after several treatment sessions over 4–6 months.

Early improvements may be noticed after several sessions, with more visible changes typically observed over 3-4 months as the hair growth cycle progresses.

This is generally not advised. Hair loss is often multifactorial, and exosome therapy typically works best as an adjunctive treatment alongside other therapies.
